DADS notified case management agencies (CMAs) and direct services agencies (DSAs) of changes regarding submission of documents for dental services to receive an evaluation for a dental treatment plan in the Community Living Assistance and Support Services (CLASS) program.
Currently, sections 2310, 2320, and 2330 of the CLASS Provider Manual require the CMA to include a completed Form 3660 with the individual plan of care (IPC) if dental treatment including an evaluation to develop a dental treatment plan, is included on the IPC.
The CLASS Provider Manual, Appendix IV, Dental Treatment will be revised to remove the requirement for a CMA and DSA to complete Form 3660, Request for Adaptive Aids, Medical Supplies, Minor Home Modifications or Dental Services/Sedation when requesting dental services for an evaluation to develop a dental treatment plan.
Effective immediately, if an IPC includes funds for a dental evaluation by a licensed dentist costing $200.00 or less, the Texas Department of Aging and Disability Services (DADS) no longer requires the CMA and DSA to complete and submit Form 3660. All other requests for dental services will require the DSA to complete Form 3660 and the CMA to submit Form 3660 with the IPC.
DADS will amend the CLASS Provider Manual in the near future to implement the change described in this IL.
